Kindergarten Medical Forms

Kindergarten Registration Health Packet

For First Time Enrollment in a Public School

2017-2018 School Year

Required documents for Kindergarten registration are the following:

  1. Certified Birth Certificate
  1. Up-to-date KY Immunization certificate
    • 3 different types of Immunization Certificate
      • Standard Certificate
      • Medical Exemption Certificate
      • Certificate of Religious Exemption
    • Deadline: August 24, 2017
  1. Preventative Health Care Examination Form- Initial Entry (see Medical Form Tab)
    • Completion Date between August 11, 2016 to present
    • Deadline: September 28, 2017
  1. KY Eye Examination Form for School Entry completed by licensed ophthalmologist or optometrist (see Medical Form Tab)
  • Deadline: January 1, 2018
  1. KY Dental Screening/Examination Form for School Entry
  • Must be 5 or 6 years old
  • Deadline: January 1, 2018
  1. (If Applicable) ALL prescribed or over the counter medication requires a School Medication form EVERY year. Must be turned into the front office. (See Medical Form Tab)

School Health Requirements: All students enrolling for the first time in Laurel County Public Schools must have the following (as referenced in KRS 158.0335, KRS 214.034, KRS 156.160 and 702 KAR 1:160 Section 2):

  • A current Kentucky immunization certificate;
  • A physical examination completed within one year prior to or 30 days after entering school;
  • A Kentucky eye examination completed by a licensed ophthalmologist or optometrist by Jan. 1 of the year of enrollment;
  • A Kentucky dental examination completed by a licensed dentist or dental hygienist (required for all 5- or 6-year-olds enrolled in public school)

CPR Certification Courses are offered monthly by the school nurses.  You must register in advance to participate.
