HCI Meal Service Request Form- HCI STUDENTS ONLY

Laurel County Public Schools Food Service Department will provide curbside pick-up meal service for all students enrolled in Home Correspondence Instruction (HCI). Please complete this form to help us prepare meals for your child(ren).

If you have children enrolled in multiple schools, all meals can be picked up at one school. Meal Service will begin on Monday, October 12th from 9:00 am to 10:00 am. Meals will not be distributed after 10:00 am.

Meal pick-up times will be on Mondays from 9:00 am to 10:00 am. Meals will not be distributed to anyone not listed on the student's Emergency Card. ID will be required to pick-up meals. Students will be provided ten meals (5 breakfast, 5 lunch to use for the week).

By completing this form, you are opting to pick-up five breakfast and five lunch meals, per student.
